ADR Post-Decree Mediation Confidential Memoranda Form

This form is used by a self-represented party or counsel for the party participating in ADR Post-Decree Mediation/Settlement Conference with Arizona Legal Center (ALC).

Each party is required to furnish the ALC Mediators with a separate ADR Post-Decree Mediation Confidential Memoranda at least seven days prior to the scheduled Mediation.

Upon completion, DO NOT FILE this form with the Clerk of Court. Please send the memorandum via e-mail to: adroffice@superiorcourt.maricoa.gov or U.S. mail to:
