You may use the forms on this page if the following factors apply to your situation:
- You have a court order from another state regarding custody, legal decision making, parenting time, or non-parent visitation of minor children, AND,
- You want to register the order so that you may now or in the future ask the Arizona court to enforce or modify the order,
- For Arizona to have the power to change the order either:
- All parties have moved from the state where the order was issued or the court where the order was issued determines that it no longer has jurisdiction to modify the order or that Arizona would be a more convenient forum,
- You understand that in order to enforce the order, the child(ren) must be in Arizona, or will be in Arizona in the near future,
- You understand that the Court is required by Arizona law to serve notice of your request to register the order in Arizona to the other parties in the case,
- You understand that the other parties in the case have a right to request a hearing to object to registering the order in Arizona.
