- You filed papers to request the court appoint a conservator for an incapacitated adult, or for a person at least 17.5 years of age who will need a conservator as an adult AND
- You gave or will give notice to all interested parties, AND
- A court hearing is scheduled, AND
- The person to serve as conservator completed or will complete any court-ordered training before the hearing, AND
- You need the paperwork to prepare for and take to the court hearing.
