You may use the forms on this page if the following factors apply to your situation:
- You want to have the court make a permanent (longer than 6 months) appointment of a conservator for a person under the age of 18, AND
- The minor lives in Maricopa County, AND
- The parents will either sign a Voluntary Consent, OR after receiving notice of the Petition to Appoint a Conservator will not file papers or come to court to object to the appointment, AND
- You know that you do not need the court to also or instead appoint a guardian. A minor generally needs a guardian if the minor needs the care and supervision of an adult.
You should not use the forms on this page if the following factor applies to your situation:
- The minor is 17.5 years of age and will need a conservator as an adult but not as a minor. You may instead file papers for an appointment of conservatorship as an adult that will become effective at age 18.
