You may use the forms on this page if the following factors apply to your situation:
- You filed papers to request permanent (longer than 6 months) appointment of a conservator for a minor to be effective until he or she reaches the age of 18 unless terminated earlier by court order, AND
- You want to have the court appoint a conservator for a minor, AND
- You have given notice to all interested parties, or will do so, AND
- You have a court hearing scheduled, AND
- The person to serve as conservator completed or will complete any court-ordered training before the hearing (see Notice Regarding Training Requirement in packet below), AND
- You need the paperwork to go to the court hearing.
