You may use the forms and instructions in this packet only if the following factors apply to your situation:

ü The person whose death certificate you need died in Maricopa County; AND

ü You are not a person eligible to receive a copy of a death certificate without a court order*; AND/OR

ü Maricopa County Vital Records has told you that you need a court order to get a copy of the death certificate.
