• A conservatorship for a minor should be terminated and restricted funds released, because:
  • The minor has reached age 18 or was legally emancipated, or
  • The minor has died.

Do not use this packet to terminate a "conservatorship of an adult." To terminate a conservatorship ordered when the subject person was an adult, see forms to terminate conservatorship of an adult.
