You may use the forms on this page if the following factors apply to your situation :

You want to have the court appoint a guardian for a minor, and you know that you do not need the court to appoint a conservator for the minor, AND
The minor lives in Maricopa County, AND
After giving notice of the petition for guardianship to BOTH parents (if living), the parents will give permission by signing a voluntary Consent, and/or neither parent will object to the guardianship, AND
If you are not related to the minor, you already have or you will get your fingerprints taken.

NOTE: You may use also this packet if you are requesting a temporary guardianship (6 months or less).
