You may use the forms on this page if the following factors apply to your situation:

  • You are at least 18 years of age, and no longer under the jurisdiction of Juvenile Court or the Department of Juvenile Corrections, AND
  • You had a juvenile delinquency case in the Superior Court of Arizona in Maricopa County, AND
  • You do not have any criminal charges pending against you in an adult court, AND
  • You want to apply to the Court to destroy your juvenile records, and/or
  • You want to apply to the Court to set aside your juvenile adjudication(s), and/or
  • You want to apply to the Court to restore your firearm rights.

 

Note: This packet contains three (3) different applications for you to ask the Court to grant you one, two, or three different results. Depending on your situation and preference, you may use one, two, or three of the applications to address your concerns to the Court.
