• You filed papers to request the court appoint a guardian and conservator for an incapacitated adult or a person at least 17.5 years of age who will need a guardian and conservator as an adult, AND
• You gave or will give notice to all interested parties, or will do so, AND
• A court hearing scheduled, AND
• The person to serve as guardian and conservator completed or will complete any court ordered training before the hearing, AND
• You need the paperwork to prepare for and take to the court hearing.
